Criteria for referral
Any child between 2 – 16 (or up to 18 if appropriate) who has a high growth chart reading (>91st centile) or who would like to have a better relationship with food, or who would like to improve their body confidence.
Exceptions can be made for children and families who do not meet the criteria but who would benefit from attending the programme.
- You can ask your GP, School Nurse, Health Visitor, or another healthcare worker to fill in a form to refer you
- Self-referral is currently not available for this programme
- If you are under 12 years old, you will need the permission of your parent or carer to join
- If the child that you are worried about is over 12 years old, you will need their permission to join
- Give as much information as you can to your health professional. This will help them to offer you the programme that suits you best
Your child will be referred to the most appropriate service based on the details you give in the referral form. This may be the ‘X Programme’, however there is also a one-to-one service called the ‘Y Programme’. You can talk to a dietitian about which service is best for your family.
